If you would like to apply for a $1500 gift that will be paid to the accredited Arizona birth center of your choice, please complete the following:

- Personal Essay, Video, or Voice Recording:
- Why Willow Wish should grant me/us this gift
- A copy of the two most recent income tax statements for tax years 2018 and 2017 Must be submitted by the mother and her partner
- If applicable: proof of enrollment in AHCCCS
- Willow Wish Recipient Waiver

th of October 2018. This gift of $1500 will be announced by the 5th of November 2018.

We are currently accepting applications from patients at birth centers in the state of Arizona who are accredited through
