How to Apply
Applying is as easy as 1 – 2 – 3:
- Fill out the application form – 2023-2024 Application Form
- Tell us why you want us to make your WISH come true. You can write it, video record it, or send in a voice recording.
- Complete the Willow Wish Recipient Waiver
Once you have completed the three parts, bundle it all together and send it our way!

Eligibility
BIRTH CENTER GRANT
Any patient at an accredited birth center in the State of Arizona is eligible to apply for a Birth Center Grant from Willow Wish. The birth centers that are currently accredited through CABC are:
Babymoon Inn (Phoenix + Tucson), Blossom Birth & Wellness Center (Phoenix), and Willow Midwife Center for Birth & Wellness AZ (Mesa + Phoenix).

DOULA CARE GRANT
Any client of a certified birth doula in the State of Arizona is eligible to apply for a Doula Care Grant from Willow Wish. The client applies and the doula must provide proof of training or a letter of recommendation from a licensed midwife or OBGYN, as well as proof of liability insurance.
