Eligibility:
Any patient at an accredited birth center in the State of Arizona is eligible to apply for a gift from Willow Wish. The birth centers that are currently accredited through CABC are:
Babymoon Inn (Phoenix), Blossom Birth & Wellness Center (Phoenix), Babymoon Inn Tucson (Tucson), Flagstaff Women's Center (Flagstaff), and Willow Midwife Center for Birth & Wellness (Mesa).
Applying is as easy as 1 - 2 - 3:
- Fill out the application form - 2019 Willow Wish Application
- Tell us why you want us to make your WISH come true. You can write it, video record it, or send in a voice recording.
- Complete the Willow Wish Recipient Waiver...and then bundle it all together and send it our way!
